Imputation and derivation

Coder instructions
For women using sterilization (V312=6:7): if she gave the method start date as before her last birth, the months usage is calculated as the number of months since her last birth; otherwise, it is calculated as V008 - V317 (date of interview minus the CMC method start date). For women NOT using sterilization (V312<>6:7): if this is an A-core questionnaire, the calendar will be used to determine the months usage. If this is a B-core questionnaire, the literal question will be asked.

For women using sterilization (V312=6:7): if the Model A questionnaire was used (i.e., there is a calendar) and V337 >= the length of the calendar, then V337 is set to 95. If the Model B questionnaire was used (i.e., there is no calendar) and V337 > 96, then V337 is set to 96.
